Kazakhstan [1] is by far the largest of the Central Asian states of the former Soviet Union. It has borders with Russia, China, and the Central Asian countries of Kyrgyzstan, Uzbekistan, and Turkmenistan. It is the world’s ninth biggest country by size, and it is more than twice the size of the other Central Asian states combined. Its lack of significant historical sites and endless featureless steppe have put many off Kazakhstan, while many still are captivated by the emptiness and mystery of this goliath state. Modern Kazakhstan is a neo-patrimonial state characterized by considerable nepotism and dominance over political and economic affairs by President Nursultan Nazarbayev. However, it is not a severely authoritarian government as compared to bordering Turkmenistan, Uzbekistan and China and opposition is not usually sacked or imprisoned. Since independence from the Soviet Union in 1991, the Kazakh government has allowed foreign investment to flow into the capital to develop. The development of significant oil and gas reserves, particularly in the north and west, have subsequently brought a large amount of wealth to the country, though the money falls into the hands of just a few people. Nevertheless, Kazakhstan is now labelled a middle-income country, and is already classified with a high human development index. Corruption in Kazakhstan is ubiquitous compared to China, but it is not as widespread compared to other countries in the region.

In principle, health care is free. However, bribes often are necessary to obtain needed care. The quality of health care, which remained entirely under state control in 2006, has declined in the post-Soviet era because of insufficient funding and the loss of technical experts through emigration. Between 1989 and 2001, the ratio of doctors per 10,000 inhabitants fell by 15 percent, to 34.6, and the ratio of hospital beds per 10,000 inhabitants fell by 46 percent, to 74. By 2005 those indicators had recovered somewhat, to 55 and 77, respectively. Since 1991, health care has consistently lacked adequate government funding; in 2005 only 2.5 percent of gross domestic product went for that purpose. A government health reform program aims to increase that figure to 4 percent in 2010. A compulsory health insurance system has been in the planning stages for several years. Wages for health workers are extremely low, and equipment is in critically short supply. The main foreign source of medical equipment is Japan. Because of cost, the emphasis of treatment increasingly is on outpatient care instead of the hospital care preferred under the Soviet system. The health system is in crisis in rural areas such as the Aral Sea region, where health is most affected by pollution.

Kazakhstan declared its sovereignty as a member of the USSR in October 1990, and following the dissolution of the Soviet Union the country declared independence in December 1991. According to news reports from June 19, 2000 Kazakh President Nursultan Nazarbayev called for creating a fund for the protection of the Russian language. He said, “Kazakhs need the Russian language like bread, every day.” He noted as well that Kazakh is the state language in the country, while Russian is the official language used on the same terms as the state language.

Speaking at a press conference in Almaty on October18, 2000, Yurii Bunakov who is one of the leaders of Kazakhstan’s Russian minority suggested that in order to prevent further Russian emigration, the Kazakh leadership should postpone the implementation of legislation calling for all business correspondence to be conducted in Kazakh until 2003. Bunakov said that special programs should be organized to help Russian-speakers learn the Kazakh language. Bunakov also argued that more Russians should be appointed to government posts. He noted that only 8% of government employees are Russians, although Russians account for 40% of the country’s population.
